Are you needing a degree verification or similar form completed to obtain state licensure? Please email the form with your information filled out and instructions about the specific section that you need our department to complete to muot@health.missouri.edu.
Some states require us to send the completed form directly to the state licensure entity while others allow us to send the form back to you to submit with your application. In either case, we will follow up with you via email to let you know we have completed our part.
Please note that if you require an official transcript, you must request that from the Office of the University Registrar.
